Registered Nurse - Heart Failure Coordinator

Registered Nurse – Heart Failure Coordinator within Yale New Haven Health System cardiac units. The RN provides high-quality, patient-centered nursing care in alignment with the Connecticut Nurse Practice Act, ANA Scope and Standards of Practice, and the organization’s professional practice model. Responsibilities include applying the nursing process (assessment, diagnosis, outcomes identification, planning, implementation, evaluation), coordinating care across the patient and family, and collaborating with the health care team. The role emphasizes ethics, cultural congruence, leadership, health teaching, health promotion, evidence-based practice, and continuous quality improvement in cardiology services, including post-operative and interventional cardiac care, heart failure management, and patient education. A BSN is preferred; candidates must be licensed as an RN in Connecticut and maintain current BCLS certification; clinical specialty certification is highly encouraged. The position is based in Wilmington, DE, with on-site responsibilities in cardiac units and related intermediate care settings.
