Job Summary
Registered Nurse – Heart Failure Coordinator role with Yale New Haven Health System involves delivering evidence-based, patient-centered cardiovascular nursing care on inpatient units, leading and coordinating care across the health care team, and upholding ANA standards and the CT Nurse Practice Act. Responsibilities include assessing and planning care, collaborating with patients and families, providing health teaching, and supporting quality outcomes. The role requires an RN licensure in Connecticut, BSN preferred, BCLS certification within one month of hire, and experience in cardiovascular/unit-based care; facilities include specialized cardiac units and post-operative care pathways such as CABG, valve replacement, and device management (pacemakers, ICDs).
Required Qualifications
- Graduation from an accredited school of nursing
- Licensed Registered Nurse in the State of Connecticut
- BSN preferred
- Current BCLS certification or obtain within one month of hire
- Clinical specialty certification highly encouraged
