Registered Nurse - Heart Failure Coordinator in the Cardiac ICU/CTICU environments within the YNHHS health system. Responsibilities include assessing patients’ physical and psychosocial conditions, developing and documenting individualized plans of care, coordinating care with the healthcare team, evaluating patient responses and readiness for discharge, providing patient/family education, serving as a preceptor to new staff, assuming charge nurse duties, and ensuring adherence to unit policies and standards of practice. The role requires ongoing demonstration of clinical competency in neuro and cardiac procedures, airway/ventilator support, and specialized heart failure care. Licensure as an RN in Connecticut is required; BSN preferred.
