Job Summary

Registered Nurse - Heart Failure Coordinator coordinates hospital-based heart failure care. Responsibilities include assessing patients’ physical and psychosocial condition, developing, communicating and documenting individualized plans of care, implementing and coordinating plans with the healthcare team, evaluating outcomes and readiness for discharge, educating patients and families, and serving as a preceptor to new staff and a charge nurse. The role requires nursing practice within state standards, and licensure as an actively practicing registered nurse in Connecticut; BSN is preferred and Basic Life Support certification is required within one month of hire. The unit focuses on advanced heart failure management in specialized ICU settings, including devices such as LVADs and ECMO, and involves oversight of patient care across neuro and airway management considerations as part of a multidisciplinary team.

Required Qualifications

  • Graduation from an accredited school of nursing
  • BSN preferred
  • Current BCLS certification or obtain within one month of hire
  • Licensed and currently registered nurse in the State of Connecticut
  • Clinical specialty certification highly encouraged
