Job Summary
Provide home-based nursing care to support recuperation, therapy, and healing at home across four counties. Responsibilities include connecting clients to primary and behavioral health care, helping clients understand discharge instructions and prescriptions, developing immediate/acute care plans with client input, creating preventive care plans after the first 30 days with client input, conducting assessments and outreach to homes, coordinating transport as requested, documenting activities and outcomes, motivating clients in self-care, and entering data into electronic medical records to ensure services are completed.
Required Qualifications
- Credentials as Registered Nurse (RN) authorized under Health Occupations Article, Annotated Code of Maryland, with current license
- insurance or at least two years’ clinical experience
- plans to achieve credentials above
- demonstrated ability to work collaboratively in a teamwork environment
- highly developed organizational and communication skills
- Current CPR and First Aide certification
- Valid full driver’s license with a safe driving record
