Job Summary
As a Registered Nurse (RN) Healthcare Advocate with Solace, you'll work with patients throughout their healthcare journey, guiding them through difficult and complex health concerns and helping them achieve health goals while addressing Social Determinants of Health (SDOH). This 1099 contract role is 100% remote, with patient interactions conducted via video or phone. Responsibilities include learning Solace's proprietary systems, building strong patient relationships, identifying and prioritizing needs, and developing comprehensive care plans to holistically address SDOH (e.g., food resources, transportation, and in-home support).
Required Qualifications
- Registered Nurse (RN) license in good standing
- Experience working with diverse patient populations
- Technical adaptability
- Bias toward action
