Job Summary
The Registered Nurse (RN) delivers home care services including initial patient and family assessments, development and revision of care plans, administration of medications and treatments, patient/family education, discharge planning, and case management for assigned caseload. Responsibilities include patient care coordination with physicians and community health partners, documenting clinical notes, coordinating equipment and services, and supervising home health aides. Requires current nursing licensure, NLN-accredited nursing education, CPR certification, and a valid driver’s license; typically seeks at least two years’ experience with public health or home care settings.
Required Qualifications
- 1. Registered nurse with current licensure to practice professional nursing in the state.
- 2. Graduate of National League for Nursing accredited school of nursing.
- 3. Maintain a current CPR certification.
- 4. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- 5. Minimum of two years’ experience, at least one of which is in the area of public health or home care nursing is preferred.
