Job Summary
Conduct initial and ongoing physical assessments of patients and families to determine home care needs. Develop comprehensive care plans incorporating therapeutic, preventive, and rehabilitative actions while counseling patients on health instructions. Administer prescribed medications, coordinate with physicians for orders and updates, and supervise home health aide care every two weeks. Manage discharge planning and act as a case manager to coordinate patient care for assigned caseloads. Participate in on-call duties as defined by policy. Requires current RN licensure, CPR certification, and a licensed vehicle. Full-time position with considerable walking, standing, and patient transfer duties.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with a vehicle that is insured in accordance with state or organization requirements and is in good working order
- Prolonged or considerable walking or standing
- Ability to lift, position, or transfer patients
- Ability to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, or crouching
- Visual acuity and hearing to perform required nursing skills
Desired Qualifications
- Minimum of two years' experience, at least one of which is in the area of public health or home care nursing
