Job Summary
Conduct comprehensive patient and family assessments to determine hospice needs and develop care plans incorporating palliative nursing actions. Administer medications and treatments as prescribed while counseling patients and caregivers on self-care techniques, diet, and medication instructions. Coordinate patient care by acting as Case Manager, communicating with physicians regarding condition changes, and collaborating with the interdisciplinary group to meet emotional and physical needs. Complete accurate clinical notes, participate in on-call duties, and supervise ancillary personnel as required. Maintain professional knowledge through continuing education and contribute to quality assessment performance improvement teams.
Required Qualifications
- Graduate of National League for Nursing accredited school of nursing
- Registered nurse with current licensure to practice professional nursing in the state
- Must have and maintain a valid driver's license
- Maintain automobile insurance coverage
- Have access to an automobile
- Visual acuity and hearing to perform required nursing skills
- Maintains a current CPR certification
- Prolonged or considerable walking or standing
- Able to lift, position, and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, and/or crouching
- Self-directed and able to work with minimal supervision
- Must comply with all training requirements
Desired Qualifications
- Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing
