Job Summary
Conduct skilled nursing assessments and provide telephone triage advice across the community, Meng Residence, and Triage department. Update care plans, administer medications, and educate patients and families on symptom management and disease progression. Respond to urgent needs, initiate crisis care, and delegate duties to LPNs and CNAs while maintaining accurate electronic medical records. Rotate between locations as staffing requires, including weekend coverage every fourth weekend and an on-call rotation of 10–15 shifts annually. Collaborate with primary care teams and physicians to ensure high-quality patient standards in our 48-year-old hospice organization.
Required Qualifications
- Associate degree in Nursing
- RN license in the state of practice
- Valid WI Drivers' License
- Reliable transportation
- Proof of current automobile insurance coverage
- Computer proficiency (MS Office – Word, Excel, and Outlook)
- Ability to communicate effectively, both verbally and in writing
- Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment
- Ability to keep up to date with policies, practices, and regulations
- Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures
- Compliance with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity
- Satisfaction of mandatory training requirements regarding local, state, and federal regulations and/or Unity's policies and procedures
- Ability to adjust to frequently changing demands
