Job Summary
Conduct comprehensive health assessments and develop individualized care plans for clients while acting as their advocate to ensure rights and preferences are respected. Perform supervisory visits, facilitate caregiver training workshops on safety protocols and best practices, and monitor care delivery to align with regulatory standards and established plans. Maintain accurate documentation of assessments and training sessions, prepare compliance reports for management, and collaborate with physicians and therapists to coordinate comprehensive care. Stay current with Long-Term Care Insurance policies and state regulations to support agency licensure processes and identify areas for continuous improvement in care delivery and procedures.
Required Qualifications
- Valid and active RN license in New Jersey
- Proven experience in conducting thorough assessments and developing care plans
- Knowledge of regulations and compliance standards in non-medical home care
- Strong communication and interpersonal skills to effectively train and support caregivers
- Ability to work independently and as part of a collaborative team
- Empathetic, patient-centered approach to care delivery
- Excellent organizational and time-management abilities
- Commitment to continuous learning and professional development
Desired Qualifications
- Knowledge of Long-Term Care Insurance (LTCI) assessments is a plus
