Job Summary
Assess patient health care needs to identify appropriate nursing interventions, triage calls for scheduling, and analyze health assessment findings. Administer medications, vaccinations, and vital signs assessments while monitoring and reporting patient symptoms. Provide health education on disease prevention and chronic disease management, helping patients use community resources and make informed health decisions. Collaborate with the interdisciplinary team to coordinate care plans, operate medical equipment, and manage clinic supplies. Maintain accurate documentation in the Family Health Team EMR and participate in quality improvement initiatives.
Required Qualifications
- Registration in good standing with the Ontario College of nurses
- Primary Health Care experience
- Outpatient clinic experience
- Excellent oral / written skills
- Strong clinical skills
- Ability to adapt to changing clinical situations
- Autonomous and able to function with minimal supervision (after training)
- Ability to work effectively with all members of the inter-professional team
- Ability to prioritize, manage time effectively and be flexible in a very active work environment
- Experience with EMRs
