Registered Nurse

Registered Nurse (RN) providing community/home health care with scheduled visits to patients across multiple settings. You will assess patients using established tools, execute and monitor individualized care plans, and maintain up-to-date patient records. Travel to patient residences within a designated area is required; on-call coverage including after-hours is expected to ensure patient safety. The role supports autonomy and independent practice within a supportive team, with opportunities for professional development, clinical orientation, and long-term therapeutic relationships with patients.
