Job Summary
Conduct scheduled visits to care for patients of all ages in various community settings, utilizing appropriate assessment tools to evaluate individualized needs. Execute and continuously monitor care plans while engaging patients through a collaborative approach to build rapport and trust. Maintain and update professional patient records related to all patient encounters and care plans. This role requires CNO registration, a valid driver's license, and on-call coverage including after hours to ensure patient safety. Full-time scheduling includes every other weekend off. Required Qualifications
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license
- Access to a vehicle
- Required on-call coverage, including after hours
