Job Summary
Registered Nurse needed to provide comprehensive home-health nursing care per patient plan of care, including evaluation, monitoring, health promotion, coordination of services, and patient/family teaching. Responsibilities include coordinating total patient care and evaluating nursing services, conducting OASIS assessments for home care, preparing and presenting clinical records, consulting with physicians on care plan changes, coordinating services with Home Health Aide and therapists, submitting regular clinical notes and progress notes, participating in case conferences, discharge planning, and ongoing professional development to improve nursing care. Must be a graduate of an accredited nursing program, licensed as an RN in the state, with knowledge of Medicare/Medicaid guidelines and home-health documentation; 1+ year experience in community/home health or hospital setting preferred; CPR certified.
Required Qualifications
- Must be a graduate from an accredited School of Nursing
- Must be licensed in the State as a Registered Nurse
- One or more years of experience in community/home health agency or in a hospital setting is preferred
- Must have knowledge of Medicare and Medicaid guidelines
- Must have a working knowledge of home health care and the principles and techniques of professional nursing and required documentation that pertains to it
- Should be skilled in organization and in time management and have knowledge of management processes
- Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff
- Must have a criminal background check
- Must have a current CPR certification
