Job Summary
Oversee patient care in home settings by assessing needs, developing care plans, and monitoring quality for adults and geriatrics. Support caregivers and clinical staff while communicating with families, physicians, and the care team. Ensure accurate documentation, resolve concerns, and apply nursing judgment to improve outcomes. Follow company policies and state/federal regulations during 8-10 visits daily on Saturdays and Sundays. Requires an active Indiana RN license, two years of home health experience, and reliable transportation.
Required Qualifications
- Active Indiana Registered Nurse (RN) license
- At least 2 years of home health nursing experience
- Valid driver's license and reliable transportation
- Strong assessment, communication, and organizational skills
- Experience using electronic medical records (EMR/EHR)
