Job Summary
Assess patients and collect health data involving patients, families, and healthcare providers. Coordinate patient plans of care to ensure interventions are consistent, safe, timely, and appropriate. Communicate clearly with all customers in person, on the phone, and in writing. Individualize patient and family teaching based on thorough assessments to meet specific learning needs. Participate in evaluating quality and effectiveness of nursing practice through quality improvement initiatives while maintaining professional knowledge of current standards, laws, and regulations. Full-time role with 36-hour weeks, 7am–7:30pm shifts, and every 3rd weekend off.
Required Qualifications
- Successful completion of an Associate's degree or BSN from an accredited school of nursing
- Current licensure to practice nursing in the State of Illinois
- Current CPR certification from the American Heart Association
