Job Summary
Prepare GP Management Plans and Team Care Arrangements while gathering patient histories, biometrics, and chronic condition data. Coach patients on sustainable lifestyle changes and conduct preventative health assessments including heart health, diabetes risk, and 45–49 screenings. Perform clinical services such as ECGs, spirometry, ABI testing, and urinalysis to monitor eligible patients. Coordinate care between GPs, specialists, and allied health professionals while maintaining accurate clinical records and Medicare compliance. Identify eligible patients for proactive outreach and support multidisciplinary teams in delivering quality chronic disease management services.
Required Qualifications
- Current AHPRA registration as a Registered or Enrolled Nurse
Desired Qualifications
- At least two years' experience providing direct clinical nursing care
- Previous experience within primary healthcare
- Experience in chronic disease management, health assessments or care planning
- Knowledge of health promotion and disease-prevention principles
- An understanding of Medicare requirements and billing processes
- Strong written and verbal communication skills
- Excellent organisation and time-management abilities
- The confidence to educate, motivate and positively influence patients
- A collaborative approach to multidisciplinary patient care
- A genuine interest in improving healthcare quality and patient outcomes
