Job Summary
Complete initial and ongoing client assessments, evaluate conditions and home environments to develop Plans of Care, and review documentation to ensure quality and compliance. Educate and support caregivers and office staff while monitoring for changes in condition to coordinate appropriate follow-up. Collaborate with families, case managers, and internal teams to assist with branch growth and clinical operations initiatives. This role focuses on professional nursing judgment, assessment, documentation, and care coordination rather than traditional bedside nursing or routine clinical procedures. Join the nation's largest privately owned non-skilled home health agency with offices in 60+ cities and nearly 20 states.
Required Qualifications
- Graduate of an accredited nursing program (Diploma, ADN, or BSN)
- Current RN license in good standing
- CPR certification
- Valid driver's license and reliable transportation
