Job Summary

Assess individual health conditions, determine referral needs, and develop a Plan of Nursing Services within 24 hours of placement or 30 days after initial placement. Monitor medication administration, ensure choking prevention protocols are followed, and maintain weekly schedules for nursing assignments. Participate in interdisciplinary team meetings, review Quality Improvement Reports, and update medical diagnoses in the electronic health record. Ensure proper medication storage, dispose of supplies according to regulations, and report all injuries immediately. Maintain ongoing health care services, schedule medical appointments, and monitor nutritional services and vital signs. Available 24 hours daily via cell phone for medical emergencies and consultation as needed.

Required Qualifications

  • Registered Nurse licensed to practice in New York State
  • Specialized training in I/DD
  • One-year experience in community health care services
  • AMAP trainer
  • CPR and First Aid Trained
  • Available 24 hrs. / day via cell phone as per medical emergency procedure/protocol, Telephone Triage Protocol and for consultation as needed
