Job Summary
Registered Nurse (RN) – Home Health role seeking skilled RNs to provide direct patient care and case management in patients' homes. Responsibilities include obtaining medical history and assessing patient condition, performing physical assessments (vitals, mobility), educating patients and families on care plans, delivering skilled nursing care while maintaining HIPAA, documenting and submitting visit records, updating individualized care plans, coordinating with LPNs, Home Health Aides, and Personal Care Attendants, and facilitating referrals for therapy or social work. Must hold an OH RN license, with preferred experience in OASIS and WellSky (Kinnser); TB test and CPR certification required; valid driver’s license; orientation at Dublin, OH. Part-time/per diem opportunities with mileage reimbursement and PTO eligibility for full-time staff, compensation offered on a per-visit basis starting at $55 per visit; locations in Dayton area and Dublin, OH with service coverage across multiple states.
Required Qualifications
- Active RN license in OH
- Experience with OASIS and WellSky (Kinnser) strongly preferred
- Med-surg and wound care experience a plus
- Current TB test (within last 12 months) and CPR certification
- Valid driver’s license
- Must be able to attend orientation at our Dublin, OH
