Job Summary
Registered Nurse to manage Peritoneal Dialysis (PD) patients, educate patients and caregivers in-clinic for home dialysis, and provide case management including home visits and on-call support. Role blends clinical care, patient education, and coordination with interdisciplinary teams. Monday–Friday, 8-hour shifts with weekends/holidays off; location in Napa, CA. Requires RN license in-state with ADN required and BSN preferred; valid driver's license and reliable transportation. Medical, dental, vision, 401(k), PTO and professional development supported. Required Qualifications
- RN license in the state of practice
- ADN required; BSN preferred
- Valid driver’s license, insurance, and reliable transportation
- Basic computer skills (MS Word, Outlook)
- Preferred: Dialysis (PD, HHD, chronic or acute) experience
- Patient education or teaching background
- Case management or telehealth experience
- CNN or CDN certification (preferred)
