Job Summary
DaVita is seeking an organized RN to manage Home Hemodialysis (HHD) patients. You’ll educate patients and caregivers in-clinic, enabling them to perform dialysis independently at home. This role blends clinical care, patient education, and case management. Key responsibilities include training patients and caregivers on HHD procedures and self-care, managing a caseload of HHD patients with assessments and care planning, monitoring treatment progress, conducting home visits, providing on-call support, and collaborating with interdisciplinary teams to ensure safe, high-quality outcomes. Schedule: Monday–Friday, 8-hour shifts with weekends/holidays off; flexible scheduling to manage your caseload. Required Qualifications
- RN license in state of practice
- Valid driver’s license, insurance, and reliable transportation
- ADN required; BSN preferred
- Basic computer skills (MS Word, Outlook)
- Experience in dialysis (HHD, PD, chronic, or acute) preferred
- Patient education or teaching background
- Case management or telehealth experience
- CNN or CDN certification (preferred)
