Job Summary
Train patients and caregivers on Peritoneal Dialysis procedures and self-care to enable independent home treatment. Manage a caseload of PD patients by conducting assessments, care planning, and home visits while monitoring treatment progress. Provide on-call support and collaborate with interdisciplinary teams to ensure safe, high-quality outcomes. This role blends clinical care, patient education, and case management for DaVita, a leading kidney care provider transforming care globally.
Required Qualifications
- RN license in the state of practice
- Valid driver's license, insurance, and reliable transportation
- ADN
- Minimum of 12 months of RN experience
- Basic computer skills (MS Word, Outlook)
- Monday-Friday, 8 hour shifts
- No work on weekends and Holidays
- Schedule: Monday–Friday, 8-hour shifts
- Weekends and holidays off
- Flexible scheduling to manage your own caseload

Desired Qualifications
- BSN
- Dialysis (PD, HHD, chronic, or acute)
- Patient education or teaching background
- Case management or telehealth experience
- CNN or CDN certification
