Job Summary
Managed Peritoneal Dialysis (PD) patients and educated patients and caregivers for at-home dialysis. Trained patients on PD procedures and self-care, maintained a caseload of PD patients with ongoing assessments and care planning, and conducted home visits with on-call support. Collaborated with interdisciplinary teams to ensure safe, high-quality outcomes. Schedule: Monday–Friday, 8-hour shifts with weekends/holidays off; flexible scheduling to manage own caseload. RN license in the practice state is required; ADN required with BSN preferred. Requires valid driver's license and reliable transportation. Basic computer skills (MS Word, Outlook) are necessary. Preferred experience includes dialysis (PD, HHD, chronic or acute), patient education or teaching, case management or telehealth, and CNN or CDN certification. Required Qualifications
