Job Summary
Manage Peritoneal Dialysis (PD) patients while educating patients and caregivers for independent home dialysis. Responsibilities include training on PD procedures, managing a caseload, conducting assessments and care planning, monitoring treatment progress, and performing home visits. Requires RN license, ADN minimum education with BSN preferred, and basic computer skills. Preferred experience in dialysis, teaching, and case management. Flexible scheduling with weekends and holidays off.

- Reliable transportation
- Basic computer skills (MS Word, Outlook)
Desired Qualifications
- Dialysis (PD, HHD, chronic, or acute) experience
- Patient education or teaching background
