Registered Nurse

Provide competent nursing care in an Oak Street Health clinic, coordinating patient care and delivering a seamless patient experience. Responsibilities include responding to telephonic requests, performing triage and symptom management (in person, phone, or group settings), medication management with refills and prior authorizations, comprehensive patient education, monitoring care plan implementation for patients on multiple provider panels, creating welcoming environments, delegating tasks to care team members, participating in care team meetings, delivering high-quality service with responsiveness and respect, and performing point-of-care testing and phlebotomy as needed. Emphasis on collaboration with diverse care teams to improve patient outcomes, maintain patient confidentiality, and utilize EMR systems effectively.
