Job Summary
The Home Health Nurse provides one-on-one nursing care in patients' homes, including admissions for all levels of patient care with infusion and wound care; handles after-hours calls with in-home visits and provides telephone education; delivers patient/caregiver education to help patients remain at home. Requires an active Missouri RN license and a valid driver's license, with at least 1 year of professional nursing experience in the last 3 years. Evening shift; on-call responsibilities; ability to work in a fast-paced home health setting and coordinate care across families, physicians, and community resources.
