Job Summary
Coordinate comprehensive management of end-stage organ diseases and oversee the patient journey through the transplant process, including living donation evaluation and recipient care in the outpatient setting. Facilitate the transplant process by identifying candidates, managing evaluation protocols, and acting as a liaison between donor and recipient teams. Monitor patient progress post-transplant, identify complications, and coordinate discharge planning while ensuring compliance with listing regulations and organ allocation policies. Provide patient and family education regarding the transplant process, risks, benefits, and immunization requirements. Maintain comprehensive documentation of candidate progress, evaluation processes, and regulatory reporting.
Required Qualifications
- Bachelor Degree Nursing
- 2 or more years acute clinical experience
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
- current active multistate state Registered Nurse (RN) license in good standing in state in which you live
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)

- Demonstrates understanding and ability to facilitate/coordinate evaluation of potential transplant candidates
- Demonstrates understanding of post-transplant management including medications, complications, interventions and discharge planning in the coordination of recipient care
- Demonstrates the ability to collaborate effectively with multidisciplinary team
- Demonstrates knowledge of, and compliance with, policies and listing requirements
- Interprets serological results of the potential transplant candidate and their implications for transplant
- Identifies absolute and relative contraindications to transplantation
