Job Summary
The Continuum of Care nursing role focuses on coordinating the patient journey from preadmission through discharge, including early identification of clinical risks and planning for post-acute care. You will lead multidisciplinary discharge planning, liaise with medical staff and allied health professionals, identify psychosocial and post-acute needs, coordinate referrals to community services and rehabilitation, and ensure timely, safe transitions of care while aligning with NSQHS standards. Strong assessment, communication, and documentation skills are essential, with proven experience in discharge planning and care coordination in an acute healthcare setting. Based at St Vincent’s private hospital in Sydney, you will contribute to reducing length of stay and optimizing patient flow while upholding patient safety and quality improvement goals.
Required Qualifications
- current registration with the Australian Health Practitioner Regulation Agency (AHPRA) with no restrictions on practice
- minimum of three (3) years’ post-registration clinical experience in an acute care environment
- experience in care coordination, preadmission assessment, and discharge planning within an acute healthcare setting
