Job Summary
Registered Nurse – Continuum of Care role focused on preadmission assessment and discharge planning within an acute hospital setting. You will coordinate referrals to community services, rehabilitation, aged care, and other post-acute supports, collaborate with medical and allied health teams, and drive safe transitions of patient care from hospital to home or alternative settings. Requires current AHPRA registration, at least 3 years of post-registration acute-care experience, and strong skills in care coordination, psychosocial assessment, and discharge planning; proficiency with electronic clinical systems (EPR) and documentation; commitment to NSQHS standards and patient safety.
Required Qualifications
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) with no restrictions on practice
- minimum of three (3) years’ post-registration clinical experience in an acute care environment
- experience in care coordination, preadmission assessment, and discharge planning within an acute healthcare setting
- proven experience in multidisciplinary discharge planning and care coordination
- excellent communication and interpersonal skills
- proficiency with electronic patient record (EPR) systems and clinical documentation
