Registered Nurse Compassionate In-Home Care

Conduct comprehensive patient assessments, develop individualized care plans, and administer medications, wound care, and IV therapy in the home setting. Monitor patient health status, document clinical changes, and educate families on disease management and home safety. Coordinate care with interdisciplinary team members including therapists and case managers while ensuring accurate documentation in compliance with regulations. Provide compassionate end-of-life and chronic disease management support as needed. Requires an active RN license in South Carolina, reliable transportation, and proficiency with electronic health records. Flexible schedule available with work-from-home options.
