Conduct comprehensive patient assessments, develop individualized care plans, and administer medications, wound care, and IV therapy in the home setting. Monitor patient health status, document clinical changes, and educate families on disease management and home safety. Coordinate care with interdisciplinary team members including therapists and case managers while ensuring accurate documentation. Provide compassionate end-of-life and chronic disease management support. Requires an active South Carolina RN license, reliable transportation, and proficiency with electronic health records.
