Job Summary
Conduct comprehensive patient assessments, develop individualized care plans, and administer medications, wound care, and IV therapy in the home setting. Monitor patient health status, communicate with physicians, educate families on disease management, and coordinate with interdisciplinary team members. Ensure accurate clinical documentation and provide compassionate end-of-life and chronic disease support. Requires an active South Carolina RN license, valid driver's license, and proficiency with electronic health records. Flexible schedule options available for home visits in the Hilton Head area.
Required Qualifications
- Active Registered Nurse (RN) license in the state of South Carolina
- Reliable transportation and a valid driver's license for home visits in the Hilton Head area
- Proficiency with electronic health records (EHR) and clinical documentation
- Compassionate, self-motivated, and able to work independently in a home care environment
