Registered Nurse Community Care

Collaborates on a plan of care based on patient needs, evaluates and modifies the plan of care based on patient responses and attainment of outcomes, conducts medication reconciliation, and functions as a core member of the care team. Leads organization of program materials and assists in promoting care coordination; ensures timely communication with patients, families, and other health care providers. Works with other health care providers to deliver patient-centered care coordination that is safe, efficient, and cost-effective. Plans, implements, and documents patient visits; assists with community outreach educational programs related to care coordination; provides patient assessment for medical and behavioral needs and formulates a plan of care consistent with patient age and profile. Supports direct patient care coordination within Community Care and Transitional Care Programs as directed, and participates in chart documentation. Requires RN licensure, BLS, and a valid driver's license; Associate's degree is required with Bachelor's preferred. This full-time role is located in Tavares, FL, under AdventHealth, with a focus on extending healing ministry through community-based care.
