Job Summary
Assess and reassess the nursing needs of terminally ill hospice patients, providing dietary counseling, treatments, and preventive procedures. Initiate appropriate nursing procedures, observe signs and symptoms, and report unexpected changes to physicians and the interdisciplinary team. Teach and supervise family members on patient care, develop care plans in conjunction with the IDT, and maintain accurate records while safeguarding information per HIPAA. Supervise and train LPNs and hospice aides, attend team conferences, and participate in an on-call rotation. Requires considerable physical effort, independent judgment, and crisis management in a home care setting.
Required Qualifications
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state
- Minimum 2 years of experience as a registered nurse
- Active patient contact within the past three years
- Ability to work independently
- Ability to make accurate, and at times, quick judgments
- Ability to supervise others appropriately
- Ability to respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural and religious modes
- Completes Hospice training program
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
- Must provide evidence of annual TB test and other state-required tests or examinations
- Must be able to work independently, make judgments based on assessments and data available and act accordingly
- Must be flexible, innovative and possess good interpersonal skills
- Must be able to cope with mental and emotional stress and demonstrate emotional stability
- Must have a current valid driver's license
- Must have auto liability insurance
- Must have reliable transportation
