Job Summary
Collaborate with the Clinical Operations Leader to check daily referrals, assign cases, and ensure client visits comply with agency and CMS regulations. Coordinate new RN orientations, including competency completion within 90 days and first-year development plans. Provide clinical oversight for plans of care reviews and perform on-site staff reviews every six months. Proactively address practice deficiencies through coaching, supervision, and open two-way communication. Schedule and lead monthly team meetings to deliver educational, clinical, and operational information. Serve as a field RN as needed to meet operational demands. Maintain current knowledge of essential RN functions and provide ongoing performance documentation to Human Resources.
Required Qualifications
- Graduation from an accredited school of nursing
- BSN
- Five years of home health clinical experience
- Two years in a supervisor position
- Prior experience in Hospice
- Valid Vermont State Nursing license
- Valid state driver's license
- Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review
- Proof of auto insurance

- Prior experience in Hospice highly desired
