Registered Nurse – Clinic Care Coordinator

Serve as a central point of contact for patients, families, and care teams by identifying unmet medical and social needs, building individualized care plans, and supporting patients through transitions of care. Provide clinical oversight to medical assistants, ensure follow-up and medication management, and facilitate care coordination meetings and quality initiatives. Work four 10-hour shifts per week to support strong work-life balance while partnering closely with providers and community resources. Maintain an Active Washington State RN license, BLS certification, and a valid driver's license. This role strengthens the entire care system at Columbia Basin Hospital, a 25-bed Critical Access Hospital in Ephrata, ensuring patients feel supported during complex moments in their lives.
