Job Summary
Provide direct patient care and case management to an assigned caseload in the home setting, including admission of patients and developing care plans. Obtain medical history, conduct physical examinations, and interpret nursing services while maintaining confidentiality per HIPAA regulations. Evaluate care effectiveness, submit visit records timely, and coordinate total patient care by supervising LPNs, aides, and attendants. Determine needs for additional medical services and submit referrals to the Director of Clinical Services. Requires current RN licensure, OASIS and Wellsky experience, and valid driver's license. Required Qualifications
- Licensed RN
- Physical within 6 months prior to first patient contact
- Current TB results (within last 12 months) and CPR Certification
- Valid driver's license and proof of liability insurance on automobile
- Ability to attend 1/2/3-day orientation at our Las Vegas office
Desired Qualifications
- OASIS and Wellsky (Kinser) experience
- Med-surge and wound care experience
