Job Summary
Registered Nurse responsible for delivering high-quality, home-based and telehealth care for Grace at Home members, collaborating with an interdisciplinary team to conduct in-home or tele-health assessments, develop and monitor nursing care plans, deploy remote patient monitoring and patient self-reporting for high-risk chronic conditions, and coordinate transitions of care and care management services across diverse settings.
Required Qualifications
- BSN Required
- Unrestricted RN licensure in the state of hire
- BLS certification
- Unrestricted driver’s license in the state of hire
- 3 to 5 years of relevant clinical experience
- Experience with EMRs
