Job Summary
Establish and maintain a disease-specific network with community and outlying hospitals, particularly focusing on VUMC Affiliate Hospitals. Ensure timely and optimal care for acute patients using evidence-based national guidelines, while coordinating outcomes data collection and serving as the discharge point of contact. Maintain TJC disease-specific certification by collaborating with the Accreditation and Standards Department, and develop protocols consistent with current practice standards. Conduct community educational activities and lectures on risk reduction, then schedule follow-up care for all patients. Act as a liaison to track post-discharge outcomes and mentor nursing staff, graduate, and novice nurse practitioners. This role supports the Clinical Neurosciences Institute Program medical leadership and includes remote data abstraction duties alongside community outreach events that may occur off normal business hours.
