Job Summary
Provides comprehensive health services under supervision for LPN and/or Medical Liaison staff, and collaborates with providers to support client wellness in a trauma-informed care environment. Responsibilities include performing comprehensive health assessments, identifying immediate and long-term healthcare needs, making referrals, coordinating with agency staff and external providers, and ensuring documentation. Oversees and mentors LPN/Medical Liaison staff, delegates tasks, schedules staffing coverage, participates in staff hiring/orientation, and supports staff development and training. Administers injectable medications, monitors client health, maintains case records, participates in utilization review and quality assurance activities, coordinates discharge planning, educates clients on health and wellness, and engages in ongoing professional development while upholding agency mission and confidentiality requirements. Required Qualifications
- Education: Associate’s Degree in Nursing required
- Registered Nurse (RN) license required
- Valid NYS Driver’s License
- Two years of nursing experience in mental health/chemical dependency/substance use disorder-related services preferred
- Bachelor’s in Nursing preferred
- Ability to supervise LPN/Medical Liaison staff
- Experience with health assessments, referrals, and coordination of care
