Job Summary

Conduct comprehensive health assessments, identify client needs, and make referrals to local resources. Administer injectable medications, monitor health, and coordinate discharge planning while maintaining accurate case records. Collaborate with primary health care providers, community agencies, and staff to ensure coordinated care and support trauma-informed environments. Delegate tasks, build cooperative relationships, and assist in staff development and hiring. Participate in quality assurance, utilization reviews, and monthly program evaluations. Review timesheets and coordinate employee schedules to ensure daily coverage. Maintain confidentiality and adhere to all regulatory standards.

Required Qualifications

  • Minimum of an Associate's Degree in Nursing
  • Valid Registered Professional Nurse (RN) License
  • Valid and clean NYS Driver's License
  • Possession of a valid NYS Driver's license and use of a registered and reliable vehicle
  • At least two years of experience related to nursing services to individuals with mental health, chemical dependency, substance use disorder, or intellectual/developmental disabilities-related services
  • Bi-lingual in Spanish
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • An ability to work efficiently and effectively and meet deadlines
  • An ability to work under pressure
  • Excellent verbal/written skills
  • Ability to maintain confidentiality
  • Ability to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies
  • Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques

Desired Qualifications

  • Bachelor's or Master's Degree in Nursing
