Job Summary
Respond to patient needs via phone triage, conduct home visits for changing conditions, and complete weekend admissions. Perform comprehensive physical assessments and history reviews to determine hospice requirements. Initiate and revise care plans based on nursing diagnoses, incorporating patient and family goals. Administer prescribed medications and treatments while documenting pain and symptom management outcomes. Communicate patient status changes to physicians and provide health care instructions per the established plan of care. Requires a valid driver's license, local travel capability, and a minimum of two full shifts per month. On-call and PRN scheduling available.
Required Qualifications
- Graduate of an accredited Registered Nursing program
- 1 year of nursing experience in post-acute care setting
- Must be licensed as a Registered Nurse within the state in which working and must maintain licensure
- Valid Drivers License
- proof of insurance
- satisfactory motor vehicle record
- Ability to communicate effectively both verbal and written
- Proficient with Microsoft Office Suite
- Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations

Desired Qualifications
