Job Summary
Administer skilled nursing care and oversee patient-centered care plans for home health clients in accordance with physician orders and state regulations. Conduct initial and regular reevaluations of patient needs, manage medication reconciliation, and coordinate care between patients, families, and external healthcare providers. Submit required documentation including OASIS and PAR data within strict timeframes while participating in case conferences and quality assurance reviews. Take on-call duty nights, weekends, and holidays to provide after-hours supervision and triage decisions as assigned.
Required Qualifications
- Current unrestricted RN license in applicable state
- Ability to meet continuing education requirements as applicable
- Graduate of an approved school of professional nursing
- One (1) year nursing experience
- Home care experience preferred
- Current CPR certification
- Ability to secure transportation and travel as required
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
- Ability to successfully meet all background and clearance requirements
- Excellent problem-solving abilities
- Positive interpersonal communication skills
- Competent performance of technical skills according to established procedures
- Effective time management skills
- Adequate computer skills with competency in media communications
- Ability to assume responsibility and effectively supervise ancillary disciplines per state requirements
- Ability to apply professional training to critically think through patient needs
- Ability to follow physician's orders
- Ability to produce accurate records of nursing activity within agency guidelines
- Ability to maintain acceptable attendance status, per Agency policy
- Ability to take on-call duty nights, weekends, and holidays, as assigned
- Ability to act as after-hours supervising nurse under the guidance of the overall Clinical Supervisor
- Ability to immediately report to Nursing Supervisor any patient incidents/variances or complaints
- Ability to report all incomplete work assignments to Nursing Supervisor
- Ability to ensure information/instruction is given in writing to the patient/caregiver/representative
- Ability to refer cases with physician's orders to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker
