Job Summary
Assess, develop, and evaluate patient discharge planning needs while collaborating with an interdisciplinary team to identify barriers and high-risk readmissions. Ensure compliance with Condition of Participation standards and lead utilization management processes for ED, admission, and post-acute providers. Apply evidence-based practice to deliver age-specific nursing care and participate in team opportunities for improvement. Work Monday through Friday, 8:00 AM to 4:30 PM, with a remote on-call rotation of 1–3 times per year. Requires a Missouri RN license and three years of acute care experience.
Required Qualifications
- Graduate of accredited school of nursing or education equivalency for licensing
- Three years' acute care experience
- Registered Nurse (RN) Issued by Compact State
- Registered Nurse (RN) - Missouri Division of Professional Registration
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- On-call rotation 1–3 times per year
