Job Summary
RN Case Manager for PRN role at MO-SSM Health St. Joseph Hospital in Lake Saint Louis. Responsibilities include assessing discharge planning needs, developing and implementing plans with an interdisciplinary team, identifying gaps and barriers to reduce readmissions, participating in utilization and admission processes, and liaising with post-acute providers. Requires at least three years of acute care nursing experience and MO RN license; preference for case management experience and EMR familiarity. Schedule: Monday–Friday 8:00 AM–4:30 PM with weekend shifts as needed; multiple duties related to discharge planning and coordinating care.
Required Qualifications
- Graduate of accredited school of nursing or education equivalency for licensing
- State of Missouri RN license or compact state RN license
- Three years' acute care experience
- Preference for case management experience
- Familiarity with EMR systems
