Registered Nurse - Case Manager

Job Summary
Assess, develop, plan, implement, and evaluate discharge planning needs for patients to the next level of care in collaboration with an interdisciplinary team. Ensure compliance with Conditions of Participation for discharge planning; identify gaps/barriers related to inpatient and observation length of stay; assess high-risk readmission patients and identify root causes; develop plans with multidisciplinary teams for discharge to the next level of care; may handle utilization management, ED case management, admission processes, and serve as a liaison with post-acute providers (skilled nursing facilities, rehab, home health, etc.); apply evidence-based nursing practice and adhere to the ANA Code of Ethics in patient care; uphold safety and professional standards; perform other duties as assigned.
Required Qualifications
- Graduate of accredited school of nursing or equivalent licensing
- Registered Nurse (RN) license in IL/MO/OK/WI (compact)
- Three years acute care experience
- Current RN licensure in applicable compact states
- Ability to perform physical requirements (lifting up to 50 lbs, etc.)
- Discharge planning experience or familiarity with CoP discharge planning requirements
