Job Summary
RN, Care Manager leads care coordination for complex patients in a value-based, field-based setting. Owns care management for an assigned panel, drives admission and readmission prevention, and facilitates transitions of care. Collaborates with PCPs, family/caregivers, Social Workers, Behavioral Health Specialists, and other team members; conducts timely nursing assessments and patient education; documents in EHR and leads in-person interdisciplinary care planning meetings. Requires active RN license, CCM certification (or plan to obtain within 12 months), 6-8 years of nursing experience, and willingness to travel throughout the communities Oak Street Health serves.
Required Qualifications
- Current RN license in assigned state
- Bachelor degree in nursing preferred
- CCM certification required or willingness to obtain within 12 months
- Minimum 6-8 years nursing experience
- Fluency in Spanish or other languages preferred
- Strong motivational interviewing and patient activation skills
- Ability to travel within community areas
