Job Summary

Provide patient-centered case management and discharge planning in inpatient and ambulatory settings, conducting assessments, care coordination, and health education to ensure agreed-upon goals and improved patient outcomes. Utilize CarePoint and MiCare daily to identify referrals, document usage, and manage patient groups, while submitting briefing documents and attending weekly huddles for team collaboration. Coordinate with host nation and CONUS facilities, aeromedical evacuation teams, and the Theatre Patient Movement Requirements Center to support patient safety and quality of life. Maintain coverage for space "A" patients and mental health cases per AFI 44-172, ensuring timely care based on acuity. Fluently communicate in English and Japanese (N2 or equivalent) to build relationships with host nation providers.

Required Qualifications

  • Able to read, write, speak, and understand the English language fluently
  • Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook
  • One registered nurse case manager position preferable speak fluent Japanese
  • Japanese-Language Proficiency Test Certificate of Results and Score of at least N2 or equivalent certificate
  • The Government will accept a Japanese STAMP Score with a Benchmark Level of 8 (Advance-Mid) as an equivalent to the Language Proficiency Test Certificate of Results and Score of N2
  • Possess the necessary knowledge, skills, aptitude, and computer literacy to evaluate, interpret, utilize and apply medical care criteria, such as InterQual, Milliman & Robertson Care Guidelines, Predictive Modeling and Integrated Medical Management Techniques
  • HCW shall be in good standing and under no sanction or suspension by the Federal Government
  • Possess and maintain current certification in either the American Heart Association BLS for Healthcare Providers or American Heart Association Healthcare Provider Course
  • Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required
  • Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies
  • Ability to pass all background/suitability checks
  • A valid driver's license
  • ability to travel locally as needed
  • Must have graduated from a baccalaureate degree program in nursing (BSN) accredited by a national nursing accrediting agency recognized by the US Department of Education
  • Nurse applicants must be a current U.S. licensed Registered Nurse
  • Active, current, valid and unrestricted license to practice nursing in accordance with State Board requirements
  • License cannot be under investigation nor have any adverse action pending from a Nursing State Board or national licensing/certification agency
  • Fully licensed (without restriction) as a registered nurse to practice nursing autonomously within the scope of nursing credentials/educational levels
  • RN must have at least three (3) years of licensed nursing practice
  • Must be a US citizen

Desired Qualifications

  • Two (2) years' experience in Discharge Planning or Case Management
  • Certification in case management from the American Case Management Association, the American Academy of Case Management or the American Nurses Credentialing Center issued within the last (2) two years
  • Equivalent combinations of education and experience may be qualifying if the education or experience is directly related to case management
  • Equivalency combinations must be acceptable to the MTF and approved by the Contracting Officer
  • An active security clearance is preferred
  • A thorough understanding of the military lifestyle and experience working with military family members
  • Working knowledge of relevant military, state, federal, and local laws and resources
  • Highly developed written, oral, and presentation communication skills
